Clinical
Hours On-Site
For a minimum of five hours per month, one of our consulting pharmacists
will be on-site to engage in the clinical activities mentioned below.
In addition, he or she often spends an equal amount of time researching
and analyzing resident related issues off-site.
Resident
Assessment
Our past associations with the University of Toronto, Faculty of Pharmacy,
and the continuing affiliations with the American Society of Consultant
Pharmacists, have enabled us to implement a pharmaceutical care model
for resident assessments. When a facility requests a pharmaceutical care
assessment for one of its residents, our consultant pharmacist conducts
a thorough analysis of the residents medical history during his
or her clinical time at the facility. then makes a comprehensive pharmacotherapeutic
assessment. All medication-related issues, therapeutic goals, and interventions
are documented and forwarded to the physician or other members of the
interdisciplinary team. Accepted recommendations then become part of a
care plan. Ongoing monitoring of the resident ensures optimal health outcomes.

Medication
Utilization Review
We provide detailed summaries of medication utilization for the facility.
Results will be benchmarked against past statistics of the facility, provincial,
and national. The consultant pharmacist will analyze trends and implement
strategies to achieve optimal medication utilization.

Medication
Distribution Management
Our consultant pharmacist will oversee all aspects of medication storage,
control, reception, distribution, administration, and disposal at the
facility level. All medication distribution activities will strictly adhere
to provincial and legislative standards. The ongoing development of comprehensive
pharmacy policies and procedures will ensure safety and accountability
in service-critical areas.

Quality
Assurance
Our consultant pharmacist is responsible for implementing the continuous
Quality Control (CQC) program at each facility. A comprehensive set of
pharmacy audits have been developed to address every provincial and professional
legislative standard. Results are summarized and sent to the appropriate
interdisciplinary member for action and follow-up.

Educational
In-Services
Our consultant pharmacist, in collaboration with the facility, will develop
and present at least four educational in-services to staff, families,
and residents at various times each year. Selected publications and textbooks,
such as the Compendium of Pharmaceuticals and Specialties along with pharmacy
newsletters on current medication issues and long-term care trends, will
be available at each facility to aid health personnel in addressing medication-related
issues.

Committee
Participation
Our consultant pharmacist is available for participation, as an integral
member of various committees at the facility, such as the Pharmacy and
Therapeutics Committee, Infection Control Committee, Medication Advisory
Committee, and Accreditation Teams. Our consultant pharmacist will chair
the Pharmacy and Therapeutics Committee at least four times per year.
