Job Title: Clinical Consultant Pharmacist

Number of Positions: (1 F/T position or 2 P/T position)

Areas to be covered: Collingwood, Thornbury, Wiarton, Southhampton, Meaford, Midland, Penetanguishene and Beeton

Department: Pharmacy - Barrie

Reports to: Clinical Pharmacy Manager

Objectives:
1. To ensure the vision of clinical pharmacy services to MediSystem’s clients and residents.
2. To ensure the vision of MediSystem’s service is provided to our clients.
3. To assist in optimizing client relations.
4. To assist in operational efforts in particular new home openings.

Responsibilities:

Administrative Duties:

1. Completes and submits consultant reports in a timely Fashion
2. Attends quarterly meetings
3. Completes assigned tasks
4. Regular communication with supervisor

Clinical Consulting Duties:

1. Ensures regular scheduled visits are made to all assigned long term care facilities, including attending team meeting and committee meetings.
2. Completes quality assurance audits and resident assessments.
3. Maintains a strong working partnership with the management team and staff at long term care facilities.
4. Ensures regular documentation of clinical duties.
5. Maintains a professional attitude and image while representing MediSystem.
6. Assists in development and implementation of industry leading clinical pharmacy service.
7. Actively participates in a self-directed continuing education program, which is of mutual benefit to both MediSystem and employee

Qualifications and Personal Requirements:

1. Current license from Ontario College of Pharmacists.
2. Good knowledge of pharmacy operations.
3. Excellent customer service skills.
4. Good presentation skills.
5. Good time management skills.
